Our office accepts a variety of health plans. These are some that are included: AARP, AARP UnitedHealthcare, AETNA, AETNA Medicare, Anthem Blue Cross Blue Shield, Anthem MediBlue PPO, Blue Cross Blue Shield Traditional, Blue Cross PPO, Cigna, Cofinity, HAP when secondary to Medicare, Humana Medicare, Medicare, Medicare Advantage, Medicare Plus Blue, Priority Health, Railroad Medicare, TRICARE, and UnitedHealthcare Choice Plus. Please call your insurance company to verify Dr. Black’s acceptance of, or participation with, your insurance carrier. Copays are due on the date of service. If your insurance information changes at all, especially if you have Medicare and the number changes, let us know and bring your new card at your next visit with us. Medicare is not accepting claims using the patient’s old number if a new number has been issued.
