Your first visit to see Dr. Black establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and we give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. If your copay amount for a specialist (like Dr. Black) is printed on your insurance card, or if you have been given that information in the past, please bring that payment. You can also contact your insurance carrier to learn your copay amount. You can even save some time at your first visit by printing out and completing the New-Patient Forms on this website in advance of your appointment. Patients find this to be very helpful because then they don’t find themselves getting stressed, trying to fill out a form at the office but not being able to remember key information.

Dr. Black’s caring and approachable nature, along with his experience in podiatric and plastic surgery, allows him to provide excellent care and put patients at ease. He and our staff offer medical and surgical care in a relaxing and comfortable environment with respect and concern. Surgery can be accommodated in the office, in the hospital, or at an outpatient surgery center.

What to Expect

Being well-prepared for your appointment will ensure that Dr. Black has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this website. Take some time to review our Staff page and familiarize yourself with our staff. Also, please print and bring your completed New-Patient Forms, a referral from your PCP if required by your insurance carrier, your driver’s license or photo ID, your insurance card(s), your copay, and your mask. Please allow yourself 15 minutes to complete New-Patient Forms prior to your appointment if you are not able to fill them out at home. We look forward to your first visit!
